1. Permit Issuer section

An authorized permit issuer completes this section. Hazard identification and risk assessment are necessary to manage risk by identifying effective controls.
The competent permit issuer must specify details of the work, including correct location, equipment tag number, list of workers authorized to perform specified work, date and time, period validity of the permit, applicable control measures, special conditions - if applicable, the related permit reference, emergency action, etc.
The permit issuer must sign the permit after confirming the implementation of all the safety measures identified and a copy must be kept with him.

2. Permit Receiver section

The competent permit receiver signs the permit after ensuring that all controls are in place and that the site is safe for work. He communicates the hazards and controls to all workers involved and is responsible for maintaining it throughout the job.

2.1. Restoration of service or clearance

This is the written confirmation that the planned work is safely completed in accordance with the agreed conditions and that the operation of the equipment or installation can resume. This section of the work permit is signed by the recipient of the permit and given to the permit issuer to close the permit.

3. Permit close out / Cancellation

In this section of the work permit, the competent issuer who issued the permit sings to accept the return of the workplace in the same state as before work. There is also an option for the permit to be cancelled due to non-compliance or any other circumstance to ensure that no further work can be done under its authority.

4. Permit extension

This section of the work permit allows the issuer and receiver to extend the period of the permit in the event of a delay or overrun. The issuer and receiver must sign this section.

Effectiveness of the work permit system

The work permit system is a system of procedural administrative control and its effectiveness depends on the competence of the issuer, receiver and the workforce involved. This system is most effective in preventing injuries and other dangerous occurrences in organizations with positive safety cultures.
To ensure the effectiveness of the work permit system
1. The permit must be issued only by an authorized person
2. The permit issuer must be competent to identify hazards, assess risks and decide on control measures.
3. The safety measures identified must be ensured before authorizing the permit.
4. Once the permit has been signed by the issuer and the receiver, it must not be altered or modified.
5. Before, during and after the defined work, all safety measures must be observed.
6. All the workforce involved must be trained and fully aware of the permit conditions.
7. A dedicated responsible supervisor must monitor the permit throughout the work.
8. Work should be stopped immediately in case of emergency or other abnormal conditions.
9. Documentation must be retained.
