2. Permit Receiver section

2.1. Restoration of service or clearance
This is the written confirmation that the planned work is
safely completed in accordance with the agreed conditions and that the
operation of the equipment or installation can resume. This section of the work

to close the permit.
3. Permit close out / Cancellation
In this section of the work permit, the competent issuer who
issued the permit sings to accept the return of the workplace in the same state

4. Permit extension
